How do diseases change Lead II, V1 and the twelve-lead ECG?

Disease-linked findings modify morphology while rhythm controls timing.

Overview

The shared ECG renderer adds representative disease-linked findings such as atrial abnormalities, voltage/strain, bundle-block morphology, scar/injury patterns or low voltage with alternans. The same finding informs telemetry and the twelve-lead view.

Changes stay lead-specific. Poor R-wave progression and isolated anterior Q/QS findings should mainly affect chest leads; they do not require a new inferior Q wave in Lead II. Conversely, atrial shape, conduction and some ST–T changes can be clear in Lead II.

A disease may have several possible findings. Model notes in the twelve-lead view can request a new condition-linked finding. Ventricular rhythms and pacing preserve their own activation morphology instead of having an incompatible bundle block added on top.
