Severe Mitral Stenosis

Severe MS raises LA/wedge pressure upstream of the stenotic valve while LVEDP remains near normal. This baseline shows passive postcapillary pressure elevation, without pulmonary vascular remodeling.

Overview

Select Severe Mitral Stenosis under Clinical controls → Valves. The bold italic live label confirms the active selection. These are the preset’s teaching features; combined disease, rate and interventions can change the resulting numbers.

Severe MS raises LA/wedge pressure upstream of the stenotic valve while LVEDP remains near normal. This baseline shows passive postcapillary pressure elevation, without pulmonary vascular remodeling.

Wedge has a prominent a wave in sinus rhythm and a slow y descent.

The a wave disappears with atrial fibrillation.

A transmitral gradient requires simultaneous left atrial (or carefully time-aligned wedge) and LV pressures; the mean wedge minus LVEDP is not the mean transmitral gradient.

Step by step

  1. Reset interventions if you want the untreated example.
  2. Select the condition and inspect the pressure summary and relevant waveforms.
  3. Pause for annotations; add one intervention or loading change to compare the response.
