Disease states
Where to select each condition and what the scenario illustrates.
Ischemic Cardiomyopathy
Chronic ischemic LV systolic dysfunction with reduced forward output and elevated left-sided filling pressures.
Mitral Valve Prolapse
Chronic severe prolapse-related MR with modest mean LA pressure elevation, a regurgitant v wave and preserved resting forward flow. Loading maneuvers alter the modeled prolapse/MR burden.
Neurogenic Shock
Loss of sympathetic tone produces vasodilation, reduced venous return and relative bradycardia.
Precapillary Pulmonary Hypertension
High pulmonary artery pressure with a preserved wedge pressure.
Preserved
Follow the pressure change as the catheter moves through the right heart.
Restrictive Cardiomyopathy
Elevated biventricular filling pressures with rapid early filling followed by a dip-and-plateau contour.
Septic Shock
An illustrative hyperdynamic septic shock profile: low systemic resistance with high forward flow.
Severe Aortic Stenosis
Severe AS with fixed LV outflow obstruction, a narrow pulse pressure at 115/85 mmHg, and impaired LV relaxation. LVEDP is mildly elevated, wedge is near its upper reference limit, and PVR is normal at baseline.
Severe Mitral Stenosis
Severe MS raises LA/wedge pressure upstream of the stenotic valve while LVEDP remains near normal. This baseline shows passive postcapillary pressure elevation, without pulmonary vascular remodeling.
Severe Pulmonic Regurgitation
Pulmonary diastolic runoff with RV volume overload and elevated right-sided filling pressure.
Severe Pulmonic Stenosis
RV systolic pressure is much higher than PA systolic pressure across the obstructed pulmonic valve.
Severe Right Ventricular Failure
RV pump failure with an elevated pulmonary vascular load produces high right-sided filling pressures and reduced left-heart preload.
Severe Tricuspid Regurgitation
Isolated severe TR with elevated right atrial pressure and a systolic regurgitant wave. Left-sided filling pressure and PVR remain normal at baseline.
Severe Tricuspid Stenosis
Obstructed right atrial emptying produces elevated RA pressure relative to RV diastolic pressure.
Tension Pneumothorax · Obstructive Shock
Right-sided tension pneumothorax restricts venous return and pulmonary flow, causing low output and hypotension despite elevated measured right atrial pressure.
